SUBSTANCE Chlorine possesses a great affinity for hydrogen; if equal volumes of the two gases are mixed and exposed to sunlight, they combine, forming hydrochloric acid. So strong is the affinity that chlorine is also able to separate hydrogen from certain of its compounds. Chlorine can also remove the hydrogen from water, setting free the oxygen. Hydrogen chloride can be prepared by heating sodium chloride with concentrated sulphuric acid; hence the former name spirits of salt. Hydrochloric acid is a clear, colourless to slightly yellow liquid with a sharp, pungent and irritating odour. It is a strong highly corrosive acid, soluble in water and alcohol but insoluble in hydrocarbons. It fumes in moist air, but it neither burns nor supports combustion. Highly corrosive, it attacks most common metals, including iron, steel and lead, and it destroys many common clothing materials. Hydrochloric acid fumes settle rapidly toward the ground.
USES Hydrochloric acid is second only to sulphuric acid in its numerous and diverse uses in manufacturing industry and synthetic chemistry. It is involved at some stage in the production of a host of everyday articles, but its principal applications include metal pickling, gelatine, dyestuffs, casein, pharmaceuticals, synthetic rubber and metal chlorides manufacture. Other important applications of hydrochloric acid are: manufacture of phenols, fertilizers and plastics; food processing [corn syrup, sodium glutamate]; water treatment; cleaning materials for households and industries; alcohol denaturation; manufacture of amino acids.
TOXICOLOGY Its corrosiveness makes hydrochloric acid a fatal poison when inhaled or ingested. Inhalation of the vapour may cause coughing, difficult breathing, pulmonary oedema, circulatory system collapse, asphyxia due to glottic or laryngeal oedema, death. The liquid can cause severe burns to skin and eyes. Ingestion causes severe burning of mouth and stomach, nausea, vomiting, intense thirst, and diarrhoea. Corrosion of the stomach may lead to gastric perforation and peritonitis. Long-term occupational exposure has been reported to cause gastritis, chronic bronchitis, dermatitis, and photosensitization in workers. Prolonged exposure to low concentrations also causes dental discolouration and erosion.
GASTRIC JUICE Hydrochloric acid is found in the stomach lining, forming part of the stomach juices [0.2-0.5%] and important in the early stages of food digestion, particularly of proteins and fats. Excessive production of hydrochloric acid causes heartburn, and gastric and duodenal ulcers. Deficiency of hypochloric acid [hypochlorhydria] or complete lack [achlorhydria] can lead to malabsorption of minerals that are usually solubilized by hydrochloric acid [e.g. calcium and iron], as well as to chronic gastritis due to destruction of the parietal cells. The condition is associated with pernicious anaemia. Acute stress may lead to increased hydrochloric acid secretion, but chronic stress causes HCl production to decrease. Achlorhydria is a common symptom in depressive neuroses and frequently associated with mental fatigue, persistent worry and strain. Symptoms include lack of appetite, fulness after eating, gaseous eructations, and diarrhoea [more common than constipation]. More than half the people with gallstones show decreased HCl secretion compared with gallstone-free patients. Diabetics have lower secretion, as do people with eczema, psoriasis, seborrhoeic dermatitis, vitiligo, and tooth and periodontal disease. The drug cannabis actively suppresses HCl secretion. Various physical stimuli as well as emotional factors have far reaching effects on the gastrointestinal mechanism. Fatigue from any cause is likely to interfere with the digestive processes, particularly those of the stomach and small bowel. Magnesium carbonate and calcium carbonate [milk] neutralize too much gastric acid.
DEPENDENCY It is believed that conflicts centring on dependency needs are especially likely to be found in individuals who develop peptic ulcers. In support of this belief is the study of Weiner et al in the 1950s. "Relying on prior evidence of a high rate of gastric secretion in ulcer-prone individuals, as indicated in part by a high level of pepsinogen in the blood serum, Weiner and his colleagues tested a population of 2073 army draftees and chose for study those men found to be maximum and minimum risk for ulcer formation by virtue of constitutional predisposition. Membership in one or the other group did in fact predict the incidence of ulcer formation during the stress of basic training. Of perhaps even greater importance, however, is the fact that a battery of psychological tests also correctly predicted the group in which pepsinogen-level recruits would fall, and even correctly identified at an above-chance level which high-pepsinogen recruits would develop ulcers. These test results showed evidence of major unresolved dependency [in Freudian terms 'oral'] conflicts, with resultant frustration and suppressed hostility. ... The important point established by this study was that neither a high pepsinogen level nor dependency conflicts was alone responsible for the development of peptic ulcer. Taken together, however, they constituted a pathogenic predisposition that, on exposure to a special stressor [basic training], could be expected to challenge the equilibrium of a dependent individual, thus producing an abnormally high risk of ulcer formation. The dependency of the ulcer-prone individual is not always obvious at the behavioural level. In fact, such an individual may seem to be quite independent or autonomous."1
